For Youth Leaders:
Instructions: Please download the Youth Registration/Release Form to hand out to your students. You will be expected to maintain responsibility for your youth, including emergency contacts/care if needed during the youth rally.  You should have the forms/information available at all times during the rally.  Once you receive the forms back from your youth, you can register your youth group using the form below.
There may be opportunities for the youth to participate in horseback riding and a climbing wall. The release forms to participate in horseback riding and the climbing wall are available with the INDIVIDUAL YOUTH REGISTRATION FORM below.  Youth leaders will be responsible for distributing, collecting, and having these forms available at the event for their youth”.
